There are going to be different rules for the qualified witch or Wizard and the student, some sections will be the same.

Name and age are self explanatory.
Blood Status can be undisclosed for those who are not members of a known wizarding family. Blood status not in fitting with the general accepted blood status of the family needs to be addressed in the history, or will not be allowed. For example, a half blood Malfoy, a traditionally pure blood family, needs to be explained away, and obviously muggle borns cannot be from an established wizarding family.
For House/School Adult characters do not necessarily have to have come from Hogwarts, they can have come from any of the other magical schools around the world, but again, this will not be allowed if not adequately explained in history.
For each Ministry position, an explanation on how they got into said position needs to be given, and needs to be in keeping with the seniority of said position. To be Head of the Auror will require more of an explanation than being a Junior Undersecretary to a senior member of the department for magical sports and games.
Reputation is your characters own personal reputation, this will differ from family reputation in order to allow for black sheep and the like, but will also help people in knowing how the wider world would view you before they necessarily get to know you. In the same way that for example, the Ancient and Most Noble House of Black has a reputation ad has certain connotations that are not in fitting with the character Sirius Black, who broke away from his families stereotypical reputation. Large discrepancies need to be addressed in History.
Family Reputation is only an important section if you wish to make a non canon family, or be from a little known family, or if you want to change the reputation from what we know the reputation of the family as in the books. Please also keep in mind the changing attitudes of the times. Things like suspicion of muggle borns in a past setting will not necessarily be seen as negative, so it is likely that families with a bad modern reputation may not have such a bad one in the past, as they were in keeping with the thoughts of the time and are viewed negatively as they have not changed along side public opinion.
History is self explanatory. How your character got where they are, and ideally something about their educational history, anything they may have been known for at school.
