Name of Spell, very straight forward, what is your spell called, it can just be the incantation again, but a lot of spells have a name, like the summoning spell and then an incantation, such as accio.
As for the incantation, most spells in this universe seem to be based off latin, so maybe check that out as a place to start, but then that is not a set rule, you can make things up or even make it non verbal. Be creative.
Spell class is simple, what branch of magic is this spell under, transfiguration, hex, curse?
Effects: what does your spell actually do? What is it's purpose? does it injure, is it positive or negative.
Limitations: Does you spell only work in under certain conditions, does it need to be a certain time of day for it to be at its optimum efficiency, do you need to ideally be in a certain mood. the more information I get, the more I can better let people learn and use it.
Spell level: 1-8 how hard is this spell to cast?
Downsides of a failed attempt. Not all spells work every time, does it blow up in your face, make you sick up slugs, blast your wig off, if it fails.

IF you wish to create spells IC, you can do it through RP, and I will add them to a limited use spell list, where I will list the practitioners allowed to use each spell.
